Criocirugía paliativa para melanoma mucoso en tiempos de pandemia por covid-19

Resumen

Los melanomas primarios en la mucosa oral surgen de la proliferación maligna de melanocitos.Son poco frecuentes, con comportamiento agresivo y pobre pronóstico. Se presenta el caso de un paciente de 82 años con melanoma mucoso oral con compromiso metastásico ganglionar cervical derecho, pulmonar multisegmentario, de unión gastroesofágica y óseo en cuerpo vertebral de L3 con extensión al saco dural, sin respuesta a radioterapia, quimioterapia ni inmunoterapia. Se le realiza criocirugía paliativa en tumor primario cada dos a tres semanas observándose disminución notoria de la lesión tumoral, con recuperación de la funcionalidad oral y disminución del tamaño de las metástasis ganglionares en cuello.  Se presenta la técnica utilizada en el contexto de pandemia por COVID-19, el beneficio de la criocirugía, su mecanismo de acción, su función inmunomoduladora y se plantea el beneficio futuro de su combinación con inmunoterapia para el manejo primario de melanomas avanzados.
