Mycobacterium marinum and verrucous lesion that simulates squamous cell carcinoma

Abstract

A 48-year-old man was evaluated in this hospital because of a lesion in the dorsum of the third finger of the right hand that had progressed in the last two years after a slight trauma. The patient refers he works with fresh and sea water fish aquariums. The first medical impression was a common wart, treated with electrofulguration. After some time there is recurrence of the lesion, this time with small satellite lesions. The diagnosis are squamous cell carcinoma, common wart, verrucous tuberculosis and acquarium granuloma. The biopsy shows chronic granulomatous inflammation, that is negative for Ziehl Neelsen and periodic acid Shiff staining. The culture for mycobacteriae is positive for Micobacterium marinum. Treatment was given with trimetroprim/sulphametoxazol during six months and after two months the lesion disappeared. After nine months of observation the patient is asymptomatic.
